Maternal Child Health Coordination Program

MOMS provides access to prenatal care, health screenings, health education and referral services through monthly home visits. MOMS mothers and their babies receive one-on-one education and support throughout pregnancy and the baby's first year of life.

During pregnancy, the program focuses on measures to increase the mother's chance of delivering a normal birth weight, full-term, healthy baby. After birth, the focus is on promoting the infant's healthy development by:.

- Establishing a medical home

- Tracking immunizations

- Promoting nutrition

- Screening for achievement of developmental milestones

- Educating parents on early brain stimulation

- Enhancing parenting skills

- Educating parents on childhood safety
